We understand that your medical information is personal and we are committed to protecting it. The law requires us to keep your medical information private and to give you this notice describing our legal duties, privacy practices, and your rights regarding your medical information. We may use this information about your to provide you with medical treatment or services. We may disclose this information to doctors, nurses, insurance companies or other people taking care of you. We may disclose your information for payment purposes or in response to a court order, warrant, or subpoena. We may use your medical information for our healthcare operations. This might include measuring and improving quality, training, getting accreditation, certificates, licenses, and credentials we need to serve you.


We may also disclose your medical information to persons subject to jurisdiction of the FDA for purposes of reporting adverse events associated with product defects or problems, to enable product recalls, repairs, or replacements, to track products or to conduct activities required by the FDA.


You have a right to look at or get copies of your medical records. If you request a copy of your records there will be a fee of $15. Your request for us to communicate your medical information to a given person must be in writing on you Acknowledgement Form. You must notify us if you no longer wish for that person to have access to your medical information.

  • We have a 24 hour cancellation policy or you may be charged a $25 fee per missed appointment.

  • Clients may not switch previously treated areas under the same package after being treated the first time.

  • Clients may not combine offers, gift cards, monthly specials or deals.
